Kindergarten Screening/Registration

Kindergarten registration for the 2026-2027 school year is scheduled for March 17, 2026

Please contact the school office at 724-354-2131 to schedule a time for your student's registration and screening. 

 

Along with your child, please bring the following items to registration:

Student Birth Certificate (original)

Student Immunization Records

Student Health History Form

Guardian Proof of Residency (ex: driver license, utility bill, your name and address)

 

Registration Forms:

1) Pick up paper forms in the school office to fill out and bring to registration/screening

2) Print the forms below to fill out and bring to registration/screening

- OR -

3) Fill out forms here at EE when visiting during registration/screening
